Jerald “Jerry” Hoffman, 41, of Carbondale, and formerly of Anna, died Monday, Nov. 7, 2022, at Memorial Hospital of Carbondale.
He was born on Jan. 24, 1981, in Cape Girardeau, the son of Bryon and Jackie Scivally Hoffman.
